How is the Asian eyelid different?
The Asian eyelid is a unique structure that differs from the Western eyelid in many ways, and a deep understanding of the anatomy and healing process is critical as there is no margin for error. Furthermore, the Asian ideal aesthetic is different from the Western ideal, and many patients desire the eyes to be more open, longer horizontally, and with a different shape. Operating on an Asian eyelid in the same manner as a Western eyelid can lead to adverse consequences such as scarring, unintended formation or destruction of the eyelid crease, or an undesirable cosmetic outcome.
The Asian eyelid may have a full crease, partial crease, or no crease. Up to 50% of Hispanic patients have an Asian-configuration eyelid with partial or no crease. Many Asians also have an epicanthal fold which gives a characteristic appearance. This variability is caused by additional layers of fat and differences in how the deeper eyelid tissues attach to the skin. This may also cause the eyelashes of the upper or lower eyelids to scratch the delicate surface of the eye, causing tearing and a gritty sensation in the eyes. Due to these complexities it is best to find a surgeon who specializes in Asian eyelid surgery in New Jersey to ensure the best outcome possible.
How is the Asian eyelid treated?
Dr. Ramesh has super-specialized experience in Asian eyelid surgery, after treating patients both in Los Angeles, CA, and Seoul, South Korea. Surgery involves removal and/or repositioning of the skin, fat, and muscles, along with precise placement of sutures to manipulate the eyelid crease; scars are hidden such that they are invisible to the naked eye.
It is extremely important that surgery on the delicate eyelids be performed by a qualified individual as inappropriate or excessive surgery can lead to lifelong complications ranging from dry eye to total vision loss. As a member of the prestigious American Society of Ophthalmic Plastic and Reconstructive Surgeons (ASOPRS), an elite society of only 500 members specializing in cosmetic surgery of the eyelids and face, we are dedicated to safely and elegantly helping you reach your goals.

What are the possible Asian eyelid surgery techniques?
Upper Asian Eyelid Lift
The upper Asian eyelid lift is often performed to create a crease in the upper eyelid. Without a crease on the upper eyelid, the eyelashes touch the upper part of the lot, making the eyes seem smaller. An eyelid crease without unwanted fat deposits makes the eyes appear larger and more alert. However, it’s important to emphasize that no one “needs” an upper eyelid crease, so you should only pursue this procedure if you truly desire an upper eyelid crease.
An upper Asian eyelid lift may also be performed to remove excess skin from the upper eyelids. In this case, an incision is made in the eyelid crease to remove excess skin and fat deposits. However, when removing excess skin from the upper eyelid, the surgeon must follow a conservative approach because removing too much skin can cause a sunken appearance. Dr. Ramesh follows a conservative approach to eyelid lifts, ensuring the results match your desired goals.
Lower Asian Eyelid Lift
The lower Asian eyelid lift is often performed to remove excess fat pads from under the eyes, thereby reducing puffiness. Individuals of Asian ethnicity generally have more fat deposits under the eyes, leading to a puffy and tired appearance. This Asian eyelid surgery in New Jersey involves making incisions within the eyelids to remove excess fat and skin tissues. However, surgeons must be extremely cautious and conservative while performing Asian eyelid surgeries because Asian skin is less prone to wrinkles and reacts strongly to incisions.
How is the upper eyelid crease made?
When creating an upper eyelid crease, Dr. Ramesh may select one of two possible techniques. The first technique involves flipping the eyelid over and using sutures to create the desired eyelid crease. This surgical technique doesn’t involve incisions, so it also ensures faster recovery and the presence of a natural-looking fold. However, in this case, the crease may eventually be lost due to pressure, especially if the patient has thicker skin.
The second technique, known as the open technique, involves making an incision in the upper eyelid. After making an incision, the surgeon sutures the skin to the muscle responsible for pulling your eyelids open (the levator muscle). However, the surgeon must perform this technique delicately to avoid damaging the levator muscle, leading to eyelid drooping (ptosis). The incision is completely hidden, and this procedure ensures long-lasting results.
Will I be asleep for surgery?
Dr. Ramesh is typically able to perform these delicate Asian eyelid surgeries in New Jersey under IV sedation, or twilight anesthesia. This avoids the risks of general anesthesia and being put to sleep completely, while still preventing you from feeling any pain. Twilight anesthesia also allows you to participate in the surgery in a way that allows a more symmetric result.
What is recovery like?
Surgery takes between 1 – 2 hours, and patients can go home afterwards. Usually, patients have bruising and swelling around the eyes for 1 – 2 weeks, and pain is minimal. Most patients are ready for social events in 2-4 weeks, and “wedding-ready” by 3 months. Strenuous activities must be limited for the first week, and patients may return to normal exercise and other activities after 7 days.
No matter the approach, our goal is for you to look more youthful and refreshed, without any appearance of having had surgery. Eyelid surgery, upper or lower, generally does not cause visible scarring after surgical healing is complete. Complications after surgery are extremely rare, and patients are typically very satisfied after the procedure.
